Certification Verification Tool

The HPCC Certification Verification Tool is a free resource to assist the public and organizations requiring verification of a health care professional's certification status. Names of individuals currently certified by HPCC are available within the following page. Information provided via this service include the name of the certified individual, home city/state, and current certification status. HPCC provides verification for ACHPN®, APHSW-C®, CHPCA®, CHPLN®, CHPN®, CHPNA®, CHPPN®, and CPLC® certification status but cannot provide primary source verification for the educational preparation or professional experience of a specific individual.

If a written verification letter is needed please email your request to [email protected] and include:

• The name of the certified nurse including the middle initial
• Certification number if available
• Mailing address if available

The certification verification letter will include:

• Name of the certified nurse as it appears in our database
• Certification number
• Appropriate credential
• Date the certification began
• Date the certification will expire
